Elder Care Homecare Services

Elder Care - Domiciliary Care - Care for Dementia - Alzheimer's Care
Cancer Care - Palliative Care - End of Life Care

Caring for elderly people and providing them with the best possible home care services enables them to make choices about their quality of life as they grow older. Life expectancy is increasing across the board but in order for elderly people to enjoy this increased longevity to the full it is important that their homecare services are designed to support their long-term wellbeing.

"Our Health, Our Care, Our Say" (Department of Health 2006) confirmed that people want support when they need it and they expect it quickly, easily and in a way that fits into their lives. Here at Crossroads Care Coventry & Warwickshire we have been providing elder care services in a way that fits into people's lives since 1974.

Care for Dementia Sufferers and Home Carer Respite

Elder Care - Domiciliary Care - Care for Dementia - Alzheimer's Care
Cancer Care - Palliative Care - End of Life Care

Dementia is a progressive illness that manifests a range of symptoms including forgetfulness and short-term memory loss, which gradually become more noticeable as time goes by. The progression of dementia brings with it confusion, disorientation and an inability to reason and, in many cases, a change in the dementia sufferer's personality.

Crossroads Care Coventry & Warwickshire provides a range of home care services specifically designed to support carers of people with dementia. Our Care Support Workers are trained to understand the problems faced by home carers looking after a loved one with dementia and are able to support the varied needs of the person with the condition.

Specifically in Rugby, Nuneaton, Bedworth and North Warwickshire, these dementia homecare services are funded by Warwickshire Social Services through the Short Break Contract. In Coventry this service is funded through a specialist grant from Coventry Health to support home carers and we also provide domiciliary care services through a specialist contract with Coventry Social Services.

Alzheimer's Care Services

Alzheimer's disease is a progressive illness that is unique to the individual and is by far the most common cause of dementia. Memory, speech and communication will be affected leaving the individual to rely on non-verbal communication prompts, such as body language and facial expressions. Individuals with Alzheimer's disease experience gradual disruption of mental abilities caused by the structural changes in the brain.

Crossroads Care Coventry & Warwickshire provides a range of home care services specifically designed to support carers of people with Alzheimer's. Our Care Support Workers are trained to understand the problems faced by home carers looking after someone with Alzheimer's disease and are able to support the varied needs of the person with the condition.

Stroke Rehabilitation Care Services

Elder Care - Domiciliary Care - Care for Dementia - Alzheimer's Care
Cancer Care - Palliative Care - End of Life Care

This is a specialist home care service in Coventry to support people recovering from a stroke. The aim of the service is to continue the rehabilitation work at home which had been undertaken by the person themselves and the Stroke Rehabilitation Team during the initial period following their stroke. Goals are set to encourage the return to activities formerly undertaken 'pre stroke' and our Care Support Workers have been trained by, and work closely with, the stroke rehabilitation team to encourage stroke survivors to achieve maximum independence.

Post Hospital-Stay Homecare Services

Elder Care - Domiciliary Care - Care for Dementia - Alzheimer's Care
Cancer Care - Palliative Care - End of Life Care

The aim of the post hospital homecare service is to prevent unnecessary re-admissions to hospitals or to residential and nursing homes. Coming home after a stay in hospital can be rather scary, causing anxiety both for the patient and their family and friends. At times like this, qualified support from a Care Support Worker can go a long way to help build confidence, improve convalescence and encourage greater independence following a period of constant care and attention.

Crossroads Care can give you that additional support to help you settle back in at home. We can provide Practical Home Care Services which could include help with shopping, cleaning, laundry or food preparation, or we can help with personal care including washing, dressing or assistance with medication.

End of Life Palliative Care Services

Elder Care - Domiciliary Care - Care for Dementia - Alzheimer's Care
Cancer Care - Palliative Care - End of Life Care

This is a specialist Coventry-based home care service funded by Coventry Health to support those people wishing to return home when active medical intervention is no longer an option. The service aims to provide practical support to the person with care needs and to give their home carer a respite break.

Crossroads Care is committed to providing flexible, responsive and individually tailored end of life and palliative care to people in their own homes. Our Care Support Workers play a vital role in helping people to approach the end of their life with compassion, dignity and respect whilst receiving homecare services that are focused on their own personal circumstances.

Emergency Homecare Services

Are you in immediate need of a Care Support Worker and assistance in your home? We can provide someone to meet your care needs at short notice and assist at times when you are in need of urgent help. Carers Response Emergency Support Service (CRESS) is an emergency service designed to give home carers peace of mind.

Many carers worry about what would happen to the person they care for if they themselves were involved in an emergency or were suddenly taken ill. CRESS is a membership scheme which works with home carers and the people with care needs to set up an emergency plan which outlines what would happen if the carer was suddenly unable to care for the person in need.

Respite Care Services for Home Carers

Support for Carers - Respite Services - Recharge Your Batteries
Me Time -
Short Breaks - Relax and Enjoy

As much as you love the person you are caring for, as a Home Carer, you yourself will need looking after sooner or later and Crossroads Care can provide you with the much-needed short break and respite from caring. Our well trained and highly qualified Care Support Workers will take over the reins and give you the chance to recharge your batteries and have a bit of "me time" for a change.

Home Based Respite Care Services

This is when a Care Support Worker comes to your home and takes the place of the carer for an agreed amount of time to give the home carer a break. Whilst the carer is away the support worker can provide any of the services detailed earlier whilst giving the home carer the security and peace of mind in knowing that everything is safe and sound at home.

Overnight Respite Care Services

This is when a Care Support Worker comes to you home and stays overnight usually from the hours of 10pm to 7am (but this can be negotiated) and provides whatever personal homecare support is needed. This then allows the carer to have an undisturbed good night's sleep.
